1. Download the enrollment form, flyer and fee agreement.

2. Fill out the:

     Enrollment form

     Fee agreement

     Allergy Form (if needed)

     Discipline Form

3. Give the filled out paperwork to the Campus Kids Site Director at your school along with the registration fee.

4. You can also scan and email the enrollment paperwork to campuskidsllc@yahoo.com

5. You will get a confirmation email when the paperwork is received.
